The YouthSource Case Manager oversees file and case management for all enrolled and assigned youth, and acts as the main and centralized information holder for the youth. The Case Manager works collaboratively with all other YSED staff to identify the best plan for youth to identify and reach their goals.  The Case Manager is also responsible for determining youth/program eligibility, maintaining documentation requirements, in depth case management services, and data input. YouthSource Case Management requires intensive case management which includes: outreach, facilitation of workshops, home visits, re-engagement, among other tasks.  The Case Manager will work with youth to identify resources and services most suitable to address youth barriers. Case Managers are responsible for tracking progress of youth from the program period to the last quarter of follow-up.


  • Oversee implementation of services for each participant.
  • Meet regularly with participants, consistently assessing needs and strategies to address barriers.
  • Complete case plans and other documentation following program policies and protocols.
  • Provide and maintain appropriate data and participant files.
  • Provide transportation vouchers and other supportive services. Collects and verifies all timesheets.
  • Works with youth to create case plans to ensure youth completes all required program components.
  • Meet participant and/or families in a variety of settings including but not limited to: schools, worksites, home, court, other community based organizations.
  • Assist with placement in education track or employment. Helps create a resource library of information on education and training programs, internships, and other post-program opportunities.
  • Meet individual and programmatic benchmarks and outputs.
  • Report all data into JobsLA database and other data collection sources in a timely manner.
  • Assist in development and coordination of special assignments and programs.
  • Attend community events, forums, and other events that relate to the goals and objects of the program.
  • Participate in staff meetings, webinars, and trainings, as required.
  • Maintain a professional attitude, confidentiality, and respect cultural differences.
  • Handle crisis and/or emergency situations as needed.
  • Complete all other duties as assigned.
  • Must be available to work some evenings and weekends.


  • Bachelor’s degree in social work or related field strongly preferred.
  • Three (3) years’ experience providing case management services.
  • Two (2) years’ working in a non-profit organization environment.
  • Experience working with youth from at-risk environments, ages 16-24.
  • Excellent oral and written communication skills; ability to make group presentations and facilitate group activities.
  • Assist with Center duties, which may include but is not limited to front desk coverage.
  • Be comfortable and assist with program outreach, which includes but is not limited to: canvassing, phone calls, presentations, partner meetings.
  • Ability to work in a fast-paced, dynamic environment.
  • Ability to work effectively with minimal supervision, prioritize multiple tasks, manage competing priorities, meet deadlines—all with attention to detail.
  • Superior computer and organizational skills with significant experience in Microsoft Office and Google Drive.
  • Must have access to reliable transportation to arrive to work site at scheduled time consistently for frequent travel between sites.
  • Must be 21, have a valid California Driver license, and have a good driving record as evidenced by submitting to HR a California Driving History report before hire, by each anniversary date, and at any time requested. (need only insert into JDs that will have to drive the van).
  • Evidence of Automobile Liability Insurance if authorized by Program Director to transport clients in personal vehicles.
  • Experience working with individuals with diverse ethnic backgrounds.
  • Demonstrated ability to work effectively as a member of a team.
  • Must pass a criminal background check (LiveScan fingerprinting) prior to beginning employment with children.
  • Must present proof of negative Tuberculosis skin test (Mantoux) prior to beginning employment dated within 60 days prior to start date.
  • Bilingual; English/Spanish preferred.
