Overview

Youth Policy Institute, under the YouthSource & Education Department (YSED)is currently seeking a Case Manager for its Pathways to Justice Careers for Youth (PJC) grant. The YouthSource & Education Department supports in-and out-of school youth with needed skills to transition into adulthood with careers, education and/or career training. The PJC Program, under the YouthSource & Education Department (YSED), will target in-school youth who are at risk of dropping out of school or at risk of involvement in the criminal justice system, or both, between the ages of 16-21. The program will provide eligible youth with case management, mentoring, career exploration, training leading to industry-recognized certiticate, post-secondary/job placement services, and follow up services. 

 

The Case Manager will work as a part of the PJC team to support a total of 250 youth, ages 16-21. This individual will work with the highest need students and families to develop strength-based solutions and to provide services that include: assessments, case management, career planning, supportive services coordination, mentorships, follow up, among other relevant services. The Case Manager will assess needs, create a service plan, and ensure progress is made towards achieving goals.

 

 

Responsibilities

Responsibilities may include but are not limited to:

 

  • Oversee implementation of services for each participant.
  • Assess participant needs.
  • Complete assessments, plans, and other documentation following appropriate procedure and protocols.
  • Provide and maintain appropriate participant data and files.
  • Participate in staff meetings, monthly trainings and other department programs as requested.
  • Travel as necessary, transporting participants to appointments, court, etc.
  • On occasion, meet participant and/or families in a variety of settings including but not limited to: schools, worksites, home, court, other community based organizations.
  • Provide transportation vouchers and other supportive services following established program policies and protocols.
  • Meet individual and programmatic benchmarks and outputs.
  • Assist with program placements and program outcomes.
  • Maintain open communication with Program Manager and other YSED staff, which should include regular updates, attendance at program meetings, participant successes, and challenges.
  • Conduct outreach and service coordination activities.
  • Handle crisis and/or emergency situations as needed.
  • Track and provide transportation vouchers and other supportive services to participants.
  • Collect, enter, and manage programmatic databases, and/or any other applicable program-related database.
  • Complete required reports and update Program Manager in a weekly, bi-weekly, and/or monthly basis.
  • Participate in staff meetings, monthly trainings, department programs, professional development trainings as required.  
  • Assist in development and coordination of special assignments and programs.
  • Maintain a professional attitude, confidentiality, and respect cultural differences.
  • Work collaboratively with other staff.
  • Handle and complete all other duties as assigned.

Qualifications

 

  • Bachelor's degree in social work or related field strongly preferred.
  • Experience working with youth from at-risk environments, ages 16-24.
  • Demonstrated ability to establish and maintain effective relationships with school personnel, after school personnel, program officers, evaluators, parents, students and community professionals.
  • Ability to provide leadership and coordination of day-to-day programming.
  • Must have independent judgment and problem solving ability.
  • Excellent oral and written communication skills; ability to make group presentations and facilitate group activities.
  • Ability to work in a fast-paced, dynamic environment.
  • Ability to work effectively with minimal supervision, prioritize multiple tasks, manage competing priorities, meet deadlines—all with attention to detail.
  • Superior computer and organizational skills with significant experience in Microsoft Office and Google Drive.
  • Must have access to reliable transporation to consistently arrive at work site at scheduled times and for frequent travel between sites.
  • If applicable, will be required to present current California driver license, proof of acceptable driving record (DMV H-6 printout), and liability auto insurance.
  • Must be comfortable with program outreach, which may include canvassing, phone calls, and presentations.
  • Ability to travel between sites for meetings and business purposes.
  • Must pass a criminal background check (Livescan fingerprinting), with both the Department of Justice (DOJ) and the Federal Bureau of Investigation (FBI), prior to beginning employment.
  • Must present proof of negative Tuberculosis skin test (Mantoux) prior to beginning employment dated within 60 days.
  • Must have access to reliable transportation.  If applicable, will be required to provide proof of current California driver license, acceptable driving record (current DMV H6 printout), and current liability auto insurance.
